REQUEST FOR CART/LIVE-CAPTIONING OR INTERPRETING SERVICES
It is the event planner’s, department’s or individual employees' responsibility to request Communication Access Real-Time translation (CART, also called live-captioning or real-time captioning) or visual interpreting services (e.g. sign-language, cued language transliteration, etc.).
To make a request for a specific event or meeting, staff should complete this form with sufficient advance notice; preferably three (3) business days or more. Employees who use communication services should also use this form to request service providers for ongoing or one-time service requests as well.
Although attempts will be made to honor requests submitted on short notice, requests which do not allow three (3) business days increase the possibility that services will not be available or that filling a request will increase the cost.
All requests must be paid for by the University department or unit sponsoring the event or meeting. After a service provider or agency has received a request, they will communicate and coordinate directly with the event contact person listed below on this form. Event logistics required for service provision are the responsibility of the event organizer. Service providers and agencies are used to working with folks new to the process, so don't hesitate to ask them questions.
A separate request is required for each meeting or event.
