Family First Service Array: Matching Families to Services
This decision-making tool tool is designed to help caseworkers and other providers, such as community-based providers, choose services from Colorado's Title IV-E Prevention Services Plan that are best matched to child/youth and family needs. The following questions assume that your client is either a child/youth who could benefit from a service, a parent or caregiver who could benefit from a service, and/or a pregnant person who could benefit from a service.
After responding to a few questions about your client, you'll be presented with information about the services for which your client may be eligible and are available in the county or counties where your client would consider using a service.
